I don't know if the following projects specifically meet those 
requirements, but what you list is a fairly standard capability of 
customer relationship management (CRM) software, which is the category 
that the following fall under:

SugarCRM Inc. (uses PHP; MySQL)

hipergate (uses Java/Tomcat; databases other than MySQL)
http://www.hipergate.org/

openCRX (uses Java/J2EE; not MySQL for production)
http://www.opencrx.org/

Compiere (more focused on ERP; uses Java; Oracle)
