Check out munin. You can find it at http://www.linpro.no/projects/munin/ and 
in Debian (probably other distros too). It's web-based, capable of monitoring 
many hosts on different nets, and is entirely too complete for me ;)

It's also a breeze to set up in Debian.

Then there's always gkrellm if you want to go that route.
