USB/DVI/VGA KVM switch?

I am looking for a 2-port (but 4-port could possibly work if that is all that
can be found) KVM switch for PCs that has ports for (not just via software
configuration) USB, DVI, and VGA.

Finding all the ports in one box has proven challenging thus far.

Descriptions are one thing - looking at the device ports has sometimes proven
different that what is printed.
