In addition to the hardware switch, you may not have the
requested firmware around.

Do you see a line like this?

iwl3945 loaded firmware version blahblahblah

If not, it goes in /lib/firmware and should either be
installable via a package or
http://intellinuxwireless.org/?p=iwlwifi&n=downloads
