Management.  Seriously, the biggest obstacle to high availability is  
management rejecting your budget.

Then it's your data center.  If it doesn't have redundant power,  
cooling and network provider links then there's no point to investing  
in server HA.  A single fault will take out everything.

Assuming that you have money and a reliable data center then the next  
obstacle is data center constraints.  You need enough space to house  
redundant servers, storage and network.  You need enough power to run  
it all, and you need enough cooling to keep it all happy.
