Membership Database

I'm looking for a php or perl based web application that will support a
membership database of names, positions, companies, phone numbers, email,
etc...  This is for a website I am developing for fun for my profession..

MySQL interface is a must...

I've checked around a few places, can't find quite what I'm looking
for..  Any pointers?
