this is really not my week.... was this ever stupid... the disk was full,
cleaned up the disk, restarted xfs and a few other services and things
started to work again.

It seems improbable that you'd have this sort of problem without
something on the server having changed...  My initial guess would be
that someone updated the SSH software, and the configuration changed
as a result.  Or, someone updated the configuration, to make it more
secure...
