Script for political donation.

I was wondering if anyone out there can help with suggestions on how
to go about collecting donations for a political candidate.  I
volunteered to help a person collect donations from their website.  I
thought that I can just paste some code from PayPal onto their site.
However, I forgot about the need to collect personal information for
people making donations to keep a record about donation amounts for
political purposes..  Does anyone have a simple solution or know of
some canned script that I can use?  The candidates web site is written
in PHP.
