I've used mediawiki over and over again with success.  So has wikipedia ;-)

There are less featured packages and probably services that give you
access to your own small wiki, but I have always hosted my own or used
them on a shared server.  Worst case scenario if you don't want to set
it up yourself is that you sign up for a shared hosting service for as
little as $30/yr like Dreamhost, which even offers a remote SSH shell
access and other goodies like a Click to Install package of
mediawiki...
