PHP Contact Form

Hi Everyone, I am trying to put together a contact form, that includes name
(F,L), street address, email, and message, I know how to make it send the
email, but I can't for the life of me figure out how to configure the PHP to
include the name and so forth in the message.

The layout would end up being (in the email)

First name Last name

Street address information

message.

any help would be much appreciated.

let me know what other information you need to work with.  Thanks so much,
