We use RT, but reasonably customized. I've seen PerlDesk (http://www.perldesk.com/), which is $99 for 5 support staff, and it seems pretty decent as well. The installation process for RT is reasonably complex, so if you are faint of heart, you might want to test the installation process first on a test machine.
